Calcified Chronic Subdural Hematoma






Abstract
Since the report of a case of calcified subdural hematoma by Lewis, only 34 cases of such cases had been reported on literature up to 1972.
A report is presented on a patient in whom plain skull roentgenograms revealed clear-cut curvilinear calcific deposits, which proved to be contained in the substance of membranes of a chronic subdural hematoma.
Oh JS, a 64-year-old Korean male was admitted to Seoul National University Hospital on Feb.-4, 1975 in comatose state. This patient, who had a history of hypertension, suddenly became aphasic, and soon fell into unconscious state. On admission, neurological examination revealed anisocoric pupils (larger on the left side) with no light reflexes, and weaker extremities bilaterally.
Plain skull films showed above mentioned findings. Lt. carotid angiogram revealed widening of U-loop with minimal contralateral shift of anterior cerebral artery, which gave the impression of intracerebral hematoma on the left side, and another mass lesion on the right side. A left frontotemporal osteoplastic craniotomy disclosed huge intracerebral hematoma clots, which was evacuated subtotally.
On right frontotemporoparietal osteoplastic craniotomy, a hard thickened membraneous structure was encountered in the subdural space, which contained two large muddy lumps of hematoma clots. Almost all of the hematoma clots including the outer and inner membranes
were succesfully removed. The biopsy material of removed membranes proved to contain calcium deposits on pathological study. Postoperatively, the general condition of the patient aggravated, and died on the ninth postoperative day.
